What Chronic Care Management actually is?
Chronic Care Management (CCM) is care coordination that is outside of the regular Primary Care Provider (PCP) office visit for patients with multiple (two or more) chronic conditions expected to last at least 12-months or until death.
In 2015, Medicare began paying providers for CCM services furnished to Medicare patients with multiple chronic conditions. (cms.gov)
